DR. UMUT SUADİYE CLINIC PERSONAL DATA PROTECTION LAW INFORMATION TEXT

 This information text hereby has been prepared to provide information to data owners concerning processing of personal information, belonging to patients/clients, who referred to OP. DR. UMUT SUADİYE CLINIC, in accordance with Law concerning Protection of Personal Data, numbered 6698 (“KVKK”) in Turkish legislation and European Union General Data Protection Regulation (“GDPR”). Ensuring the security of Personal Data of patients/clients, is one of the priority goals of the PHYSICIAN. For this purpose, for secure processing of Personal Data of patients/clients and for prevention of all kinds of unauthorized access to or leakage of such data, all necessary security measures are taken by Op. Dr. UMUT SUADİYE in accordance with current legislation.

DATA OFFICER:

Your personal data will be processed in accordance with KVKK and GDPR by Op. Dr. UMUT SUADİYE, who conducts its activities in the OFFICIAL REGISTERED ADDRESS. Op. Dr. UMUT SUADİYE is deemed as “Data Officer” under KVKK and as “Controller” under GDPR. Op. Dr. UMUT SUADİYE has contracted a law office, specialized on Personal Data Protection as “Data Protection Officer” under GDPR and in OP. DR. UMUT SUADİYE CLINIC, the adaptation process concerning protection of personal data under KVKK and GDPR has been realized with consultancy and guidance of Specialist Law Office, provided that the responsibility for implementation is to be borne by Data Officer.

 YOUR PERSONAL DATA TO BE PROCESSED:

Your Personal Data and Special Category Personal Data will be processed by OP. DR. UMUT SUADİYE CLINIC in line with fundamental principles, laid out in Article 4 of KVKK and Article 5 of GDPR, in line with Personal Data Processing Conditions and Objectives, specified in Articles 5 and 6 of KVKK and Articles 6 and 9 of GDPR and the objectives specified in this Information Text, in compliance with law, in connection with and limited to and proportional with the processing purpose, by considering the principles of equity and transparency, in a correct and up-to-date manner, by adopting all kinds of technical and administrative measures, proposed in the legislation.

THE PURPOSE OF PROCESSING YOUR PERSONAL DATA:

Your Personal Data and Special Category Personal data, will be processed by OP. DR. UMUT SUADİYE CLINIC in compliance with KVKK and GDPR in order to create a patient file, to provide examination, preventive medicine, medical diagnosis and treatment, medical procedure, to realize surgery and care applications, to perform medical applications, to perform your controls following medical diagnosis and treatment and/or surgery, to manage possible complication processes, to establish one-on-one contact with you, to manage appointment processes, to realize patient satisfaction and request management, to fulfill legal and contractual obligations, to store information, related to your healthcare data, which must be stored in accordance with the law, for prescribed time periods, to ensure security of clinic, to obtain consultation service from other specialist physicians to provide correct treatment for you, to fulfill legal obligations in compliance with International Healthcare Tourism Legislation, to plan transfer and accommodation services for Healthcare Tourism patients, who arrive within the framework of International Healthcare Tourism, to announce new offers concerning medical treatments and applications, to inform 3rd persona concerning provided medical services, in order to conduct promotion and marketing activities within the framework of International Healthcare Tourism Legislation, to plan and manage healthcare services and financing, to fulfill obligations, arising from the legal relationship between the physician and the patient, to fulfill financial and administrative obligations, to provide technical security and to fulfill public obligations. Your Personal Data and Special Category Personal Data will be processed by OP. DR. UMUT SUADİYE CLINIC under following conditions and situations in accordance with KVKK and GDPR: Your identity data will be processed for the purposes of conduction of examination, preventive medicine, medical diagnosis, treatment, surgery and care services, to create patient file, to conduct patient satisfaction and request management, to plan transfer and accommodation services for Healthcare Tourism patients/clients, who arrive within the framework of International Healthcare Tourism.

Your contact information will be processed  to perform your controls following medical diagnosis and treatment and/or surgery, to manage appointment processes and in addition, provided that you give explicit consent, to promote, notify, announce news concerning medical applications and operations and to remind appointment dates in continuous treatments and to send greetings in special days.

Your healthcare information will be processed in order to perform your medical treatments and your operations successfully, to consult another specialist in your treatment process, when deemed necessary, to manage possible complication processes, to create a patient file, to store information, related to your healthcare data, which must be stored in accordance with the law, for prescribed time periods,  to fulfill legal obligations in compliance with International Healthcare Tourism Legislation. Your photograph, video and audio recordings will be processed by Op. Dr. UMUT SUADİYE in order to observe and manage treatment process, to ensure security of clinic and in addition, provided that you give your explicit consent, to inform and promote performed medical procedures and/or operations for 3rd Person Healthcare Tourists, who wish to be treated in our country within the framework of International Healthcare Tourism in accordance with Healthcare Tourism Incentive Legislation and may be published in INSTAGRAM, FACEBOOK, TWITTER accounts of clinic and physician, in the official website of physician and the website of clinic, in identifiable form with your face visible, in  compliance with ethical rules and law. Personal data may be published after it is rendered impossible to associate with any identified or identifiable real person even after combination with other data (anonymization). This issue will be determined on the will of the physician in accordance with the ethical rules and law depending on the nature of the area, subject to procedure. Your financial data will be processed to conduct control of your payments and to issue invoices and in case of an overpayment, to return the same. Your transfer and accommodation information will be processed in order to plan transfer and accommodation services for patients/clients, who arrive within the framework of International Healthcare Tourism. Your Personal Data and Special Category Personal Data, specified above, will be processed in line with purposes and legislation, specified above, for performance of your medical treatment and/or operation and for fulfillment of obligations, related to your treatment. In case you do not provide related personal data, legal obligations, imposed on OP. DR. UMUT SUADİYE CLINIC, which will serve as a physician in your surgery and medical treatment, will not be properly fulfilled and your surgery/treatment and recovery processes may not be successfully managed.

METHOD OF COLLECTION OF PERSONAL DATA:

Depending on healthcare service, provided by Op. Dr. UMUT SUADİYE, with your health reports, laboratory and imaging results, tests and your statements concerning other healthcare data, presented for medical evaluation concerning your treatment during your visit to the clinic for examination, treatment and/or surgery purposes, with the “Patient Information and Consent Form,” related to the treatment and/or surgery to be performed by the Physician, with the communication form, completed on the corporate website of Physician, with electronic mails, sent to corporate electronic mail address of Physician, with photographs/video recordings, taken by physician before, after and/or during the medical procedure, performed on you, on your request and when necessary, your written/audio/video messages (photographs and/or video recordings sent to Op. Dr. UMUT SUADİYE  using remote connection applications service providers (whatsapp, zoom.us, facetime, skype, messenger, google, instagram, facebook, etc.), from which you receive service by accepting their Privacy Policies and Foreign Transfer Policies and with your online audio/video calls, established over these applications.

Your personal data will be collected with direct messages, sent to the profile account of Op. Dr. UMUT SUADİYE in various social media accounts (instagram, youtube, facebook, twitter, linkedin, etc.), currently used by you by accepting their Privacy Policies and Foreign Transfer Policies and whose servers are domiciled abroad and/or by making comments to the posts, with your information, transferred with consent for automatic processing via panels, such as “contact us” or “learn more” in the advertisements and promotions by Op. Dr. UMUT SUADİYE in various social media accounts (instagram, youtube, facebook, twitter, linkedin, etc.), currently used by you by accepting their Privacy Policies and Foreign Transfer Policies and whose servers are domiciled abroad, with your request to join the existing wireless internet network (Wi-Fi) in the Physician’s clinic as a guest user, with logging devices (firewall), used within the framework of technical measures, adopted in line with legislation, with the security cameras, used to ensure physical security of workplace within the framework of legitimate interest, and will be processed in accordance with KVKK and GDPR for proper and complete fulfillment of legal and contractual obligations by Op. Dr. UMUT SUADİYE during examination, preventive medicine, medical diagnosis, treatment and care services.

PURPOSE, METHOD AND LEGAL GROUNDS FOR PROCESSING OF YOUR PERSONAL DATA:

 Your Personal Data may be processed by real or legal entities as “DATA PROCESSORS,” authorized by Op. Dr. UMUT SUADİYE, by taking verbal, written records, camera and photographic records, by adopting necessary technical and administrative security measures and by keeping logs in physical and electronic medium and by obtaining your separate explicit consent, in cases, mandated by related provisions of KVKK and GDPR.

CASES, REQUIRING YOUR EXPLICIT CONSENT :

In accordance with Article 6/1/a of GDPR and Article 5/1 of KVKK, in the following cases, your Personal Data may only be processed on your explicit consent: delivery of SMS messages and/or e-mails and establishment of mobile contact with you for the purposes of introduction, announcement and notification of changes, related to medical services, offered by the Physician, may only be possible on your explicit consent. Sharing your photograph and/or video records, taken during and/or following medical procedure in order to observe the progress of medical treatment, on the social media accounts (instagram, youtube, facebook, twitter, linkedin, etc.) and corporate website of Op. Dr. UMUT SUADİYE may only be possible on your explicit consent. Since sharing of your data via service providers, with servers, domiciled abroad, will be deemed as “foreign transfer” your explicit consent will be sought for such situations due to requirement of “explicit consent” condition for foreign transfer.

However, in the following cases, since a foreign transfer by Op. Dr. UMUT SUADİYE will be out of question, YOUR EXPLICIT CONSENT WILL NOT BE SOUGHT AND YOUR EXPLICIT CONSENT WILL BE DEEMED TO HAVE BEEN OBTAINED BY THE SERVICE PROVIDER, SUBSCRIBED BY YOU; in case you send written/audio message and/or photograph/video recordings to Op. Dr. UMUT SUADİYE using service providers domiciled abroad, which are currently used by you after acceptance of their Privacy Policies and Foreign Transfer Policies (whatsapp, zoom.us, facetime, skype, messenger, google, instagram, facebook, etc.) or in case you request to obtain remote examination/diagnosis services in this way, you’ll be deemed to have provided your explicit consent for processing of your Personal Data and Special Category Personal Data, shared with Op. Dr. UMUT SUADİYE, in line with the Personal Data Protection Information Text and Privacy Policy of Op. Dr. UMUT SUADİYE and to be contacted via such applications, in accordance with the policies of these applications, which you accepted in advance, you will be deemed to have granted explicit consent to said applications for transfer of your data to abroad. In case you request to be contacted via electronic mail providers (such as hotmail, gmail, yahoo, etc.), whose Privacy policy and Foreign Transfer Policies were accepted by you and whose servers are domiciled abroad, you will be deemed to have accepted the processing of your Personal Data and Special Category Personal Data, shared with Op. Dr. UMUT SUADİYE in said messages in accordance with PDP Information Text and Privacy Policies and you will be deemed to have provided explicit consent to said applications for the transfer of your data to abroad, in accordance with their respective policies, which were previously accepted by you. In case you refer to the advertisements and promotions of the Physician, published on social media service providers, domiciled abroad (such as facebook, instagram, Google, Whatsapp, etc.), your personal data, for which you gave your consent for automatic processing and foreign transfer by accepting Privacy Policies and Foreign Transfer Policies of these social media service providers, will be transferred to Op. Dr. UMUT SUADİYE by related social media service provider and will be processed and stored by the Physician in line with Physician’s own Information Text and Privacy Policies, contained in the links, given in the application. In these cases, you hereby agree to processing of your Personal Data and your Special Personal Data within this framework and to be contacted by Op. Dr. UMUT SUADİYE using your specified contact information.  In case you send a direct message to the profile account of Op. Dr. UMUT SUADİYE in social media accounts (instagram, youtube, facebook, twitter, linkedin, etc.), currently used by you by accepting their Privacy Policies and Foreign Transfer Policies and whose servers are domiciled abroad, you will be deemed to have agreed to processing of your personal data in line with Physician’s own Information Text and Privacy Policies, contained in the links, given in the application, and to be contacted over related application.

CASES, IN WHICH YOUR DATA MAY BE PROCESSED WITHOUT EXPLICIT CONSENT:

  In accordance with Article 9/2/h of GDPR and Article 6/3 of KVKK, in order to perform examination, medical diagnosis, surgery, treatment and care services, your healthcare data, deemed as Special Category Personal Data, will be processed by the Physician, who is under the obligation of confidentiality under the law. In accordance with Article 6/1/b of GDPR and Article 5/2/c of KVKK, in order to perform your controls following medical diagnosis, surgery and treatment procedures, to manage possible complication processes, to establish one-on-one contact with you, to manage appointment procedures, your personal data will be processed by Op. Dr. UMUT SUADİYE without your explicit consent. In accordance with Article 6/1/f of GDPR and Article 5/2/f of KVKK, in line with our legitimate interests, your Personal Data will be processed by Op. Dr. UMUT SUADİYE without your explicit consent in order to ensure the security of clinic, to ensure patient satisfaction and request management. In line with legal obligations under Article 6/1/c of GDPR, Article 5/2/a and 5/2/ç of KVKK, your Personal Data will be processed without your explicit consent in the following cases: Creation of a patient file, storage of information concerning healthcare data, which has to be stored under related legislation, to control your payments and to issue invoices, fulfillment of tax payments, performance of obligations under Ministry of Health legislation, performance of obligations under Healthcare Tourism legislation, provision of data security, performance of legal obligations before judicial bodies, performance of administrative obligations before Administrative Institutions and Bodies.

 TRANSFER OF YOUR PERSONAL DATA AND THE PURPOSE AND RECIPIENTS OF SUCH TRANSFER:

 Your Personal Data and Special Category Personal Data, collected within the framework of the conditions and purposes, specified under Personal Data Processing Conditions, given in Articles 5 and 6 of KVKK, numbered 6698 and Articles 6 and 9 of GDPR, may be transferred, in accordance with Articles 8 and 9 of KVKK and Articles 45 and 49 of GDPR, to the following 3rd persons, for the purposes of performance and improvement of examination, preventive medicine, medical diagnosis, medical procedure, surgery, treatment and care services,  obtaining consultation service from other specialist physicians when necessary, fulfillment of legal obligations in compliance with International Healthcare Tourism Legislation, planning transfer and accommodation services for Healthcare Tourism patients, who arrive within the framework of International Healthcare Tourism, establishment of contact with patients, planning and management of healthcare services and financing, conduction of promotion and marketing activities within the framework of International Healthcare Tourism Legislation, fulfillment of obligations, arising from the legal relationship between the physician and the patient, performance of financial, legal and administrative obligations, provision of technical security and fulfillment of public obligations, to the extent sufficient for such purpose, by signing necessary confidentiality contracts and by adopting all administrative and technical measures in compliance with legislation.

Within this framework, your Personal Data and Special Category Personal Data, processed by Op. Dr. UMUT SUADİYE, will be transferred to its employees, affiliates and subsidiaries, other specialists for consultation purposes, private hospitals, suppliers, financial consultants, tax and financial consultants and auditors, legal consultants, Database (Server) Providers, “Clinical Management System Software” Service Provider, Translators, Foreign Promotion Marketing Consultant, Support Management System (DYS) authority Data Protection Personnel, IT Consultant, Travel Agency, Medical Device/Prosthesis Provider Companies, Public Institutions and Establishments, authorized by law and Judicial Bodies.

 DURATION OF PERSONAL DATA PROCESSING:

Your Personal Data will be stored by Op. Dr. UMUT SUADİYE to the extent and as long as it is mandatory to legally store the same. Documentation and storage obligations arise from relevant local norms, related to commerce, tax and healthcare legislation. In the table, given below, the storage periods for your Personal Data, are given.

AGE LIMIT FOR DATA SUBJECTS WITHIN THE FRAMEWORK OF LEGISLATION:

 Within the framework of KVKK, patients/clients, who are at least 18 years of age, may provide consent in person for processing of their personal data. The consents for patients/clients, who are younger, must be given by their legal representatives. Within the framework of GDPR, citizens of European Union member states patients/clients, who are domiciled in the European Union, may give consent for processing of their own personal data if they are at least 16 years of age or over the age limit, imposed in their own countries. The consents for patients/clients, who are younger, must be given by their legal representatives.

 WITHDRAWAL OF CONSENT:

 In case you give consent to Op. Dr. UMUT SUADİYE for processing of your Personal Data, we hereby notify you that in accordance with Article 6/1/a and Article 5/1 of KVKK, you may withdraw your consent any time. When you notify your withdrawal of consent to the following contact address as specified following, your consent will be withdrawn.

 RIGHTS OF DATA SUBJECT IN ACCORDANCE WITH GDPR:

 Your Personal Data as Data Subject are also protected under GDPR. The rights of data subjects, who are subject to GDPR (European Union citizens or persons, domiciled in European Union), are as follows (Article 15 of GDPR): By filing an application before Op. Dr. UMUT SUADİYE, the data subject is entitled to learn whether or not personal data concerning him or her are being processed and if processed, the details, given in Article 15 of GDPR.  Right to Rectification (Article 16 of GDPR): Data Subject is, at any time, entitled to file an application and to have personal data, in possession of Op. Dr. UMUT SUADİYE, which have been changed. Right to Erasure (Article 17 of GDPR): Data Subject is entitled to file an application and to have personal data, in possession of Op. Dr. UMUT SUADİYE, to be erased. In case of realization of conditions, specified in Article 17 of GDPR, your Personal Data will be erased by Op. Dr. UMUT SUADİYE without delay. Right to Restriction of Processing (Article 18 of GDPR): In case of a contest concerning accuracy of Personal Data, Data subjects are entitled to request restriction of the use of data until confirmation of accuracy of Personal Data by Op. Dr. UMUT SUADİYE. In case Data Subject requests erasure of Personal Data due to unlawful Personal Data processing activities, he/she is entitled to request the restriction of the use of data until fulfillment of request.

In case Op. Dr. UMUT SUADİYE has no need for Personal Data for the purposes of processing, the Data Subject is entitled to request restriction of the use of data. In cases, where Data Subject contests processing activities under Article 21/1 of GDPR, Data Subjects are entitled to request restriction of the use of data until it is confirmed that the legitimate reasons of Op. Dr. UMUT SUADİYE in data processing prevailed over the legitimate interests of Data Subject. Right to Data Portability (Article 20 of GDPR): If technically feasible, Data Subject is entitled to file application to request transfer of Personal Data, in possession of Op. Dr. UMUT SUADİYE, to another controller. However, this right may only be exercised when data processing is based on consent or is required under contract. Right to Object (Article 21 of GDPR):Data Subject is entitled to object processing of Personal Data on the basis of reasons, related to his/her particular condition, including profiling under paragraphs (e) or (f) of Article 6(1) of GDPR. In case Op. Dr. UMUT SUADİYE fails to demonstrate a strong legitimate reason, such as establishment, use or protection of a legitimate right, which prevails over the interests, rights and freedoms of Data Subject, it cannot process Personal Data. Where personal data is processed for direct marketing purposes, the Data Subject is entitled to object at any time to processing of Personal Data concerning him or her for such marketing, which includes profiling. In case Data Subjects objects to processing of Personal Data for direct marketing purposes, Personal Data will not be processed for these purposes.

 RIGHTS OF DATA SUBJECT IN ACCORDANCE WITH KVKK:

 The rights of real persons, whose Personal Data is processed, in accordance with Article 11 of KVKK, are as follows: To learn whether personal data, related to them, is processed or not, if yes, to request information concerning the same, to learn the purpose of processing of personal data and whether these are used according to the purpose, to learn the third persons within the country and abroad, to whom personal data is transmitted, in case the personal data is processed incorrectly or incompletely, to request correction of the same and within this framework, to request notification of third persons, to whom the data is transmitted, while processed in compliance with the Law and other legal regulations, in case the reasons, requiring processing are eliminated, to request the deletion and/or destruction of personal data and within this framework, to request notification of third persons, to whom the data is transmitted, concerning transaction, to oppose any unfavorable results through analysis of processed data exclusively by automatic systems, in case a damage is suffered due to illicit processing of personal data, to request indemnification of damage.

In case there are nay rights or requests you wish to exercise, you may submit your written application, containing the right you wish to exercise among the rights of data subjects, given in GDPR and in Article 11 of KVKK, bearing your physical signature and together with documents to verify your identity, in person by visiting the address of Op. Dr. UMUT SUADİYE, given in corporate website, may send the same via notary or sign the same with secure e-signature and deliver to our e-mail address or deliver the same using other methods, specified in KVKK. In accordance with “Communiqué concerning Rules and Procedures for Application to Data Officer,” your application must contain name-surname, signature, ID Number/Passport Number, Provisional ID Number, residence or workplace address, e-mail address, telephone and fax number and requested issues. Depending on the nature of request, Op. Dr. UMUT SUADİYE will conclude the request free of charge as soon as possible and within maximum thirty (30) days. However, in case the process requires an additional cost, the fee, specified in the tariff, determined by Personal Data Protection Board, will be charged.

 CONSENT AND AGREEMENT:

 When you read and accept this Information Text, you will be deemed to have agreed, declared and undertaken that you have full and complete information concerning data processing activities, conducted by Op. Dr. UMUT SUADİYE, you learned about your rights, granted under KVKK and GDPR and you give consent to PROCESSING of your Personal Data by Op. Dr. UMUT SUADİYE in accordance with this Information Text on your free will. After reading Information Text, you are free not to accept it. Op. Dr. UMUT SUADİYE has the title of “DATA OFFICER” under KVKK and “CONTROLLER” under GDPR. Respectfully,

PATIENT EXPLICIT CONSENT TEXT 

This text has been prepared in accordance with first clause of Article 5 of the Law concerning Protection of Personal Data, numbered 6698, in order to provide consent by related person, to processing of personal data by Op. Dr. UMUT SUADİYE on the basis of informed consent, given on free will. By accepting this Authorized User Database Registry and Electronic Commercial Communication Consent/Explicit Consent Text, you, reserving your rights arising from related legislation, hereby accept delivery of data, audio and visual electronic commercial messages, sent to your e-mail address, telephone number and other electronic applications for commercial purposes, the use, storage and sharing of shared information for this purpose. In accordance with related legislation, in particular the Law concerning Regulation of Electronic Trade, numbered 6563 and Law concerning Protection of Personal Data, numbered 6698, by recording the withdrawal of approval/explicit consent, the content of commercial electronic messaging and other records concerning the message, to be presented to related Ministry and competent public institutions and establishments when necessary, will be stored within the framework of the rules, specified in the information text in compliance with Storage and Destruction Policy of our clinic starting from the date of expiry of the approval, except in case of any disputes. In case you wish to withdraw your approval/explicit consent and you do not want to receive messages, sent to you for campaign promotions or information, you may use the opt-out procedure in the messages, sent to you. Your option to opt-out from permitted database, will not terminate the contracts, made with our clinic and you’ll continue to receive services. Explicit consent is a right, which is closely bound to person and this explicit consent, given to Data Officer for sharing of your Personal Data, may be withdrawn at any time.  However, since withdrawal procedure will only yield prospective consequences, all activities, based on explicit consent will be discontinued starting from the moment of receipt of withdrawal notification by data officer.

A- I hereby declare that I authorize in advance for storage, transfer, collection and recording, processing and storage of my Personal Data by Op. Dr. UMUT SUADİYE and data processors (registered employees of PHYSICIAN) and I hereby agree, declare and undertake that I give my consent to the sharing as follows:

B- In accordance with “Law concerning Protection of Personal Data”, numbered 6698 and Regulation concerning Processing and Provision of Confidentiality of Personal Healthcare Data”  I hereby agree, declare and undertake that I give consent to  processing, storage and sharing of my personal data, special category personal data, healthcare data and all kinds of personal data, including ones, which are identified or identifiable, submitted by me in verbal/written and/or electronic medium, by Op. Dr. UMUT SUADİYE and I hereby agree, declare and undertake that I give my explicit consent to processing and sharing of my personal data and special category personal data, specified in “Law concerning Protection of Personal Data”, numbered 6698 and Regulation concerning Processing and Provision of Confidentiality of Personal Healthcare Data,” detailed below. Data, containing information concerning identity of person, which clearly belongs to an identified or identifiable real person, which is processed partially or fully automatically or manually as a part of data registry system; Documents, such as driving license, identity card and passport, including information concerning name-surname, ID Number, nationality information, mother’s name-father’s name, and information such as tax number, SSI number, signature information, etc. Data, which clearly belongs to an identified or identifiable real person, which is processed partially or fully automatically or manually as a part of data registry system; such as telephone number, address, e-mail address, fax number, IP address. Data, which clearly belongs to an identified or identifiable real person, which is processed partially or fully automatically or manually as a part of data registry system; records, taken on entry to physical premises and during the stay in physical premises and documents, camera recordings, fingerprint recordings and recordings at security checkpoint.

Data, containing information concerning identity of person, which clearly belongs to an identified or identifiable real person, which is processed partially or fully automatically or manually as a part of data registry system; The data, mentioned in Article 6 of Personal Data Protection Law (e.g., healthcare data, including blood type, biometric data, etc.).

C- Being fully information concerning how the personal data, belonging to me and my dependents, will be processed by Dr.  OP. DR. UMUT SUADİYE I know that my Personal Data and Special Category Personal Data will be processed and will be transferred to Republic of Türkiye Ministry of Health, affiliated institutions and establishments of Ministry of Health, private information management systems and management systems, attached to Ministry of Health. In addition, for the purposes of creation and monitoring of appointments, planning and management of healthcare services and financing, conduction of storage and archiving operations, follow-up of requests/complaints, conduction of medical diagnosis, treatment and care services and provision of information to authorized persons, institutions and establishments, I hereby declare that I give my explicit consent to;

1- Processing, storage, transfer, collection, recording, processing, storage of my personal data and special category personal data by Op. Dr. UMUT SUADİYE and data processors (Dr.  OP. DR. UMUT SUADİYE employees),

2- The transfer of the same to your suppliers within Türkiye or abroad (for example, sworn financial consultants and legal consultants, information technologies, service providers, catering and porter service providers), to the extent it is related and limited to services, provided in our clinic;

3- The transfer of my identity information, images and health data to domestic healthcare institutions and establishments in order to obtain a second opinion in the diagnosis and treatment of my health condition;

 4- Along with the personal data, specified above processing of my medical photographs, taken by the physician during procedure, to obtain photographs, recordings during the procedure and to store the same,

5- To share the photographs in social media. By giving this explicit consent, you hereby give your explicit consent for sharing your images, taken via cameras and mobile phones by our clinic, only in the social media accounts and website of our Clinic. Your photographs, taken concerning the procedure, are taken to record pre- and post-procedure status and to determine whether the healthcare service is rendered properly. Among the taken photographs, only the ones, for which you have explicit consent, will be shared.

6-To provide information concerning appointments and campaign information via social media or tools, such as SMS, and the notification of sharing of my financial data to the e-mail address,

7- The transfer to the foreign healthcare institutions, physicians and healthcare professionals, rendering consultation/laboratory services, limited to the provision of medical consultation/examination/test, in case I give my consent to such consultation/examination/test,

8– Explicit Consent for Transfer of Special Category Personal Data to Family Members and Other Approved Persons In accordance with the provisions of related legislation, in particular the Law concerning Protection of Personal Data, numbered 6698, Patient Rights Regulation, Regulation concerning Personal Health Data, other than the cases of medical necessity and necessity of transfer on court order (e.g., Republic of Türkiye, Ministry of Health, Republic of Türkiye, Social Security Institution, courts, establishments, involved in patient transfer/transportation, hospital/patient information management system firms), the transfer of my health data (e.g., test results, examination information, appointment information and general information concerning my health status), to the following family members / relatives, companions, attorney or legal representative and other authorized third persons, specified below.
